Provides a variety of support to aid in adult and juvenile participants' successful completion of the diversion program including: coordinating both internal and external services, identifying and fostering professional relationships with other community-based providers, connecting diversion participants with community resources and providers as needed, participating in multi-disciplinary meetings, providing educational advocacy, maintaining client records by documenting all participant contacts and tracking progress, facilitating restorative justice practices through the District Attorney’s Office, and assisting with adult and juvenile intake appointments. May assist with data collection and reporting related to grant reports and applications.

The Diversion Case Manager may work with both adult and juvenile clients with a range of needs including those needing high acuity mental health navigation, youth charged with sex offenses, and low risk clients needing to engage with restorative justice practices.

Generally, the hiring range is $58,835-$73,544 annually. Qualifications, education and experience as it relates to the position will be taken into consideration when determining hiring salary.

ESSENTIAL DUTIES AND RESPONSIBILITIES:  (The following examples are illustrative only and are not intended to be all inclusive.)

  • Coordinates with the diversion team to identify appropriate restorative justice practices for participants and facilitates restorative justice practices including community circles.
  • Collaborates with clinical staff to create individual plans outlining program requirements for diversion participants.  
  • Works in conjunction with program participants to promote successful completion of their individualized program goals.  
  • Coordinates shared treatment planning with external treatment providers, executes appropriate waivers and releases of information, and ensures compliance with treatment and program requirements.  
  • Leads on-going psychoeducation groups for the lowest-need program participants in collaboration with a licensed therapist.
  • Researches and identifies community-based resources as needed, conducts due diligence on community-based resources and establishes professional working relationships between agencies to foster ease of access and collaboration.
  • Participates in multi-disciplinary meetings.
  • Helps families to understand educational options and coordinates with schools on transitions.
  • Creates case management notes in participant electronic files and all other required paperwork or documentation, maintains participant confidentiality, tracks participant progress and helps to determine program violations and successful completion.
  • Works closely with the clinical team to ensure appropriate level of supervision for each client, determines frequency and modality of participants contacts with the program.
  • Educates client and family regarding the justice system and consequences of further law involvement.  
  • May assist in tracking and collection of restitution.
  • Collects and reports various case-related statistics. 
  • May assist with data collection and reporting related to grant reports and applications. 
  • Formulates case conceptualization and case presentations to supervisor, co-workers and outside agencies.
  • Responds to clients in crisis and makes referrals as needed.
  • Attends mandatory staff meetings and bi-annual retreats, various conferences and professional training seminars.
  • Presents material about program services and/or relevant topics to schools and community service agencies.
  • May serve on one or more community service/agency committees.

MINIMUM QUALIFICATIONS:

EDUCATION and/or EXPERIENCE:  

  • Bachelor's degree, or equivalent experience, in social work or closely related human services field. 
  • Two (2) years of progressively responsible case management duties involving direct service provision with adults, juveniles and families. 
  • Experience facilitating groups or collaborative meetings and familiarity with handling large caseloads (40+) preferred. 
  • A combination of education and experience may be considered.
  • Bi-lingual (Spanish-English) preferred. 

CERTIFICATIONS, LICENSES, & REGISTRATIONS: This position requires successful completion of a criminal background check including fingerprinting through a national database.

In the event of an emergency/disaster in or near the County, all County employees are expected to make every effort to be available to assist the County Manager, Elected/Appointed Officials and Department Directors to ensure the continued operation of any and all necessary County functions.  This may mean being available to perform additional duties and hours beyond what is normally required.  In the event that an exempt employee does work more than 40 hours a week in support of County operations during an emergency, such employee may receive overtime or other appropriate wage compensation in accordance with existing County policies or at the discretion of the County.
