- SUMMARY
The Youth Aid Panel (YAP) case manager is responsible for providing holistic, trauma-informed support to high-risk youth and their families to reduce the barriers that prevent access and completion of the YAP program. The YAP case manager is also responsible for providing the same trauma informed support to individuals identified by the Montgomery County District Attorney’s Office to ensure vulnerable populations receive appropriate care and resources to reduce the risk of system involvement.

- QUALIFICATION REQUIREMENTS
Required
- Bachelor's degree in social work or related field.
- Experience working with at-risk youth and vulnerable populations.
- Good networking and communication skills.
- Experience providing case management.
- Ability to work in a changing environment.
- Valid driver's license and car insurance by date of hire.
- Excellent organizational skills.
- Good computer and database skills.
- Experience working with multi-disciplinary teams.
