POSITION SUMMARY:
The program is fast paced serving 150 homeless people a day. This is the only centralized day center in Downtown San Jose where our un-housed community receive service navigation and basic needs. The service navigator is part of a team comprised of a program manager, housing navigators, service navigator, health professionals, and a director. This is a full-time, in person, position, M-F, 9:00AM – 5:00PM, located at our drop-in office called The Window.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide individuals their mail, register them to receive mail, purge and file mail.
- Assist United States Postal Officers bring mail inside the building including UPS, FedEx, GLS, DHL, and Amazon.
- Assist clients with becoming housing document ready, e.g., ID and Birth Certificate.
- Assist clients accessing CCSCC John XXIII Gardner Healthcare Clinic for the homeless.
- Assist clients registering and accessing CCSCC Senior Nutrition hot meal program.
- Provide Service Navigation connecting clients with on-site services, services within CCSCC agency, and services in the community.
- Update Homeless Service Resource hand-outs and postings on The Window.
- Assist clients using the telephone landline and or lap top regarding appointments, Social Security Department, Employment Development Office, Santa Clara County Social Services, Parole/Probation check-ins, Street Outreach Housing Case-Managers, Shelter Placement Hotline, Suicide Crises Hotline, Substance Use Treatment, Behavioral Health Services.
- Provide basic needs, hygiene, sandwiches and emotional support, building trust during interactions and client behavioral de-escalation as needed.
- Refer eligible homeless individuals to the county’s reentry center for assessment for CCSCC Bridges of Hope (BoH) reentry case-management services.
- Provide Homeless verification with Office of Supportive Housing (OSH) leading to permanent supportive housing.
- Welcome Probation and Parole officers into The Window office area facilitating communication as they check-in on their clients.
- Assist street outreach housing case-mangers connect with their clients.
- Assist Bridges of Hope with case-management as needed including BSCC reentry.
- Attend meetings and trainings. Maintain professionalism and confidentiality.
- Coordinate volunteers donating sandwiches and hygiene including assisting volunteers with a push cart, unloading donations.
- Assist San Jose State University Nurses curbside wellness tables, including other curbside projects.
- Assess Homeless status via VI-SPDAT assessment tool as needed.
- Provide data entry, grant reports, monitor BoH gift cards in the safe.
- Supervise volunteers, Interns, Community Health Workers providing evaluations at the end of service
- Maintain a safe, healthy, learning environment, keeping area organized and clean.
QUALIFICATIONS:
EDUCATION AND EXPERIENCE
- Any combination of High School Diploma, GED, Bachelors, Community Health Worker or other allied health fields and two years relevant experience with target population.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
- Ability to communicate with a very diverse vulnerable population.
- Reach 5ft to put mail away in mail boxes.
- Proficiency in business applications, Microsoft Office suit, Excel and Data Entry
- Ability to lift 25 lbs.
OTHER QUALIFICATIONS:
- Criminal background check via livescan fingerprint.
- Must have TB test performed and submit results.
- Automobile, valid driver’s license and auto insurance per agency policy; or have access to reliable transportation.
WORKING CONDITIONS:
- Work will be conducted indoors and outdoors at off sites related to the agency within Santa Clara County.
HOURS AND OTHER CONDITIONS:
- Standard office hours are Monday through Friday, from 8:00 a.m. to 5:00 p.m.
- This is a full-time, non-exempt position. Some evenings and weekends may be needed due to program needs.
- Funding for this position is approved up to June 2028.
