Case Manager 

(Grant Funded Position)

Under the guidance of the Uintah County Attorney's Office, you'll provide certified case management services to adults in Uintah County's criminal justice programs. This involves delivering evidence-based case management and supervision to help participants succeed. You'll assess individuals for language barriers, cognitive ability, and emotional state, and gather crucial information from court documents, clinical recommendations, and various collateral sources (e.g., caregivers, law enforcement, treatment programs, DCFS, substance use tests, etc.). You'll also coordinate with relevant parties like parents, law enforcement, and treatment providers to monitor progress. A key part of your role will be to develop and adjust case action plans based on court orders, clinical evaluations, risk assessments, and client interviews, ensuring a smooth transition to treatment services as needed.


We're looking for candidates with a bachelor's degree in social work, social science, criminal justice, or a related field. Alternatively, a Utah Case Management Certification or four years of equivalent work experience will be considered.


You'll need a strong understanding of behavioral health practices, criminogenic risk factors, and treatment modalities, and how to develop case action plans using evidence-based assessment reports and coordinated care principles. Familiarity with language and cognitive abilities, the criminal justice system, and relevant laws and policies is also required.


Key skills include crisis intervention, clinical and court documentation, evidence-based interviewing techniques, and proficiency with basic computer programs. You must be able to deliver evidence-based case management services to adults in the criminal justice system, their families, and other key individuals. This involves identifying physical and mental disabilities, assessing substance use symptoms, communicating clearly and effectively, maintaining thorough records, and efficiently coordinating multiple tasks.


A valid Utah Driver's License and a successful full background check are mandatory.
