This position will assess resident and family needs, attend quarterly and annual care plans, and provide individual and group support services to residents, family members and staff. This role will link resident and family to sources of information or facility and community resources to aide in the resident/family adjustment, rehabilitation, discharge, in house moves, or terminal care process, and assist with financial assistance needs (Medicaid, legal services).
This position adheres to and maintains Providence Mount St. Vincent and the nursing department policies and procedures. Persons in this position also display and promote the Mission of the Sisters of Providence through the values of respect, excellence, compassion, justice and stewardship.

Required qualifications:
- Bachelor's Degree in Social Work or
- Master's Degree in Social Work.
- 2 years of experience appropriate to job responsibilities.
Preferred qualifications:
- Geriatric and/or medical social work.
