Job Description

POSITION SUMMARY:

The population served are those released from State Prison with a focus on those who are un-housed. The position works in a team setting with case-managers, housing navigators, data analyst, program manager, and project director. The primary focus is attaining housing for a shared case load of 60 clients implementing a housing first model for all enrolled clients providing service navigation and basic needs. The position is located inside our drop-in office called The Window. The position is fast paced.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Within 1 week of client program enrollment, establish mail service for at least 25 BSCC clients a year, registering them to receive mail, purge and file mail, so they have an address, which will reduce barriers to accessing benefits and seeking employment.
  • Respond to immediate, urgent basic needs of clients to promote their stabilization; e.g., emergency supplies, toiletries and sandwiches building trust during interactions and emotional support.
  • Provide Service Navigation connecting clients with on-site services and services in the community.
  • Assist clients with becoming housing document ready, e.g., ID and birth certificate.
  • Assist clients with shelter placement while becoming housing ready.
  • Provide homeless verification with office of supportive housing (OSH) leading to permanent supportive housing.
  • Welcome Parole officers into The Window office area facilitating communication as they check-in on their clients.
  • Assist Bridges of Hope reentry case-manages who are transferring clients to BSCC reentry case-load.
  • Maintain good communication with agencies, county and private service providers. Attend meetings and trainings. Maintain professionalism and confidentiality.
  • Assist volunteers donating sandwiches and hygiene products with a push cart.
  • Maintain stock of emergency supplies for clients to access.
  • Maintain a safe, healthy, learning environment, keeping area organized and clean.
  • Enter client statistics into AWARDS data base responding to data needs regarding what the CCSCC BSCC data analyst needs.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE

  • High School diploma, GED, or Undergraduate Student and two years relevant experience with target population or a combination of education and experience.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

  • Ability to communicate with a very diverse vulnerable population.
  • Reach 5ft to put mail away in mail boxes.
  • Proficiency in business applications, Microsoft Office suit, Excel and Data Entry
  • Ability to lift 25 lbs.

OTHER QUALIFICATIONS:

  • Criminal background check via livescan fingerprint.
  • Must have TB test performed and submit results.
  • Automobile, valid driver’s license and auto insurance per agency policy; or have access to reliable transportation.

WORKING CONDITIONS:

  • Work will be conducted indoors and outdoors at off sites related to the agency within Santa Clara County.

HOURS AND OTHER CONDITIONS:

  • Standard office hours are Monday through Friday, from 8:00 a.m. to 5:00 p.m.
  • This is a part-time, non-exempt position (15 hours per week). Some evenings and weekends may be needed due to program needs.
  • Funding for this position is approved up to June 2028.
